3 12 hours ago Full Job Description Juvenile Detention Counselor (Case Management) Job Summary Under broad supervision, plans, managers, and evaluates social services programs.
Administers and coordinates the services and program activities in dependency, mental illness, youth development, or physical or developmental disability programs.
Job Duries:
Responsibility 1 Performs and/or directs intake services to youth Collects and assesses in-depth information about a client's situation and functioning to assess his/her abilities, needs, and eligibility for services Performs or directs admission and orientation of youth in accordance with policy Thoroughly review collateral information and other background information to determine appropriate services as a program participant in program services.
Administers juvenile assessment instruments according to standardized procedures.
Accurately scores assessment results and appropriately reports conclusions for evaluation, treatment planning and treatment progress.
Develops appropriate treatment objectives from test interpretations.
Conducts initial and quarterly family/guardian contact to discuss systems of care and progress of the youth.
Responsibility 2- Provides counseling to individuals, groups or families, to effect positive change in the youth's behavior and/or social situations.
Conducts interviews to identify problems, assess mental and educational status and select applicable programs Communicates clearly and accurately to the youth the goals and objectives of the program.
Conducts individual counseling sessions with youth on a regular basis.
Conducts group sessions in accordance with the program guidelines.
Ensures appropriate group mechanics are performed and provides adequate feedback to program participants.
Demonstrates knowledge of appropriate usage of behavioral, cognitive restructuring and relapse Properly identifies high-risk situations and implements appropriate safety plan.
Documents each group session, to include concepts taught and an assessment of participants' progress, in accordance with established time frames and guidelines.
Conducts rounds daily on the living unit to ensure youth needs are identified and addressed.
Provides evening and weekend coverage to provide for the social, emotional, and behavioral needs of youth.
Responsibility 3 -Provide collaborative meetings with other agency stake holders for discharge recommendations.
Coordinates discharge plan in cooperation with team, clients, and their families Reviews client's file and properly evaluates progress in achieving therapeutic/educational goals and objectives at mid-point and upon completion of the program.
Prepares clear and accurate individualized program completion summaries for each participant for review by members of the YCRT Team.
Communicates regularly and effectively with other staff concerning progress of the client.
As needed, provides evaluations of client's problems to institution management when problems may be corrected by changes in the client's program or treatment plan, or when explanation of problems may have a bearing on professional evaluation of the client for a potential change in program or treatment.
Responsibility 4 -Leads or participates as a member of a multidisciplinary team to identify youths' strengths, resources, symptoms, and behavioral challenges to develop and monitor an effective implementation of the service plan.
Presents a complete and accurate summary of service planning needs to multidisciplinary team members at multidisciplinary meetings.
Participates in the identification of youth's strenghts and weaknesses and the selection of appropriate treatment goals and interventions.
Provides appropriate service needs input into the development of the the youth's service plans.
Initiates appropriate changes in the youth's service plan at multidiciplinary team meetings based on reassessment of identified goals and objectives.
Communicates accurately and clearly to the youth and his/her family members/significant others the identified service plan goals.
Monitors the youth's progress through interview, observation, input of team members, chart reviews, or other methods.
Accurately conveys progress information to family members/significant others as appropriate.
Writes concise, accurate, clear multidisciplinary team notes in the youth's facility programs record as required by policy.
Responsibility 5- Prepares and submits required reports and other documentation.
Counsels' youth including vocational and psychosocial adjustment counseling, throughout the rehabilitation process Communicates effectively with on-site Institutional Program Director concerning logistical needs or program changes/progress and with institutional leadership concerning security issues, etc.
Submits monthly progress report/reviews in an accurate and timely manner.
Provides appropriate input in strategy and review sessions (other managing teams), committees, and task forces.
Responsibility 6- Attends continuing educations course geared toward trends and professional development in the field of Case Management Services.
Attends seminars and professional meetings when available.
Maintains a continuing education program by attending in-service training when scheduled.
Responsibility 7- Maintain confidentiality of Case Record and release of information as required by policy.
Observes rules of confidentiality as stated by departmental guidelines in the dissemination and interpretation of tests results Provides information to educate the public, other agencies, vendors, and potential customers about case management services AGENCY
Qualifications:
Bachelor's degree from an accredited college or university in one of the areas of:
Psychology, Sociology, Social Work, Counseling or a directly related field AND Two years of case management experience.
PREFERRERD
Qualifications:
Master s degree from an accredited college or university in one of the following areas:
Psychology, Sociology, Social Work, Counseling or a directly related field.
